Chinese Medicine Treatment for PCOS
Liver Fire and Stagnation Type
Symptoms: Irregular menstruation, scanty or absent menstruation, long-term infertility after marriage, dense facial acne, breast tenderness, irritability, mood swings, excessive thirst, and dry stools. The tongue appears red with a yellow coating.
Prescription: Longdan Zhizi decoction
Longdan Cao, Zhizi, Huangqin, Chaihu, Sheng Dihuang, Cheqianzi, Ze Xie , Danggui , Baishao, Mudanpi, Mutong, Fabanxia, Chenpi, Fuling, Zhi Gancao.
Modification: If constipation is severe, add Dahuang. If breast tenderness is severe, add Yujin. For severe dysmenorrhea, add Yimucao.
Phlegm-Dampness Obstruction Type
Symptoms: Scanty menstruation, delayed or absent menstruation, irregular spotting, long-term infertility, increased vaginal discharge, dizziness, headache, chest tightness, nausea, fatigue in the limbs, excessive phlegm in the throat, sticky stools, obesity, dense body hair, and a white, greasy tongue coating.
Prescription: Danggui Cangzhu decoction
Danggui , Cangzhu, Xiangfu , Zhike , Chenpi , Guizhi , Fuling, Fabanxia, Chuanxiong , Chuanlianzi , Duzhong, Honghua , Danxingye, Gancao.
Modification: If there is weakness and soreness in the lower back and knees, add Sangjisheng and Tusizi. If there is excessive phlegm, add Zaojiaoci and Shichangpu. If there are palpable masses in the lower abdomen (enlarged ovaries, thickened capsules), add Kunbu, Xiaokucao, and E Zhu.
Qi Stagnation and Blood Stasis Type
Symptoms: Delayed menstruation, scanty or heavy menstruation, abdominal pain during menstruation that worsens with pressure, passage of blood clots, depression, irritability, insomnia, chest and hypochondriac distension, and a tongue with a dusky purple appearance.
Prescription: Danggui Baihe Decoction
Danggui, Chuanxiong, Baihe, Chishao, Taoren, Shengdihuang , Mudanpi, Honghua, Yimumao, Zhike, Xiangfu, Yanhusuo, Wulingzhi, Gancao.
Modification: If there are palpable masses in the lower abdomen and heavy menstrual blood clots, add Chaihu and Niuxi. If there is severe abdominal pain, add Chaihu and Niuxi.
Kidney and Spleen Deficiency Type
Symptoms: Delayed menstruation, scanty, pale, and watery menstruation, possible amenorrhea, long-term infertility, dizziness, tinnitus, lumbosacral soreness and weakness, aversion to cold, increased urination, loose stools, decreased libido, obesity, a pale tongue, and a thin white tongue coating.
Prescription: Dihuang Shanyao Decoction
Shoudi, Shanyao, Mudanpi, Shanzhuyu, Fabanxia, Bajitian, Rougui, Zhi Fuzi, Chenpi, Gouqizi, Fuling, Ze Xie, Zhi Gancao.
Modification: If there are blood clots during menstruation, add Niuxi. If it's a young patient, add Fupenzi, Zhi Shouwu, and Xianlingpi. If there is also phlegm-dampness, add Zhebeimu and Zaojiaoci.
