TCM Classification and Prescription for Treatment

Liver Depression and Qi Stagnation Type

Symptoms: B-ultrasound shows fatty liver, discomfort and even aching in the right rib area, belching, acid reflux, abdominal distension, and loss of appetite, among other physical symptoms.

Formula: Chaihu Baishao Decoction

Ingredients: Chaihu, Baishao, Zhike, Xiangfu, Yujin, Chuanlianzi, Yanhusuo, Danggui, Niuxi, Baizhu, Gancao, Shanzha, Yinchen, Heye.

Modifications: For sharp rib pain, add Chishao and Danshen; for fatigue, add Huangqi and Dangshen; for menstrual pain, add Yimucao.

Indications: Fatty liver developed after hepatitis, alcoholic fatty liver.

Phlegm-Dampness Obstruction Type

Symptoms: Rib distension, poor appetite, fatigue, lethargy, lack of vitality, and possibly nausea, vomiting, loose stools, abdominal bloating, and obesity.

Formula: Chaihu Banxia Decoction

Ingredients: Chenpi, Qing Banxia, Fuling, Zhuru, Zhishi, Cangzhu, Houpo, Ze'xie, Chaihu, Baizhu, Muxiang, Shanzha, Jueming, Yinchen, Heye.

Modifications: For sticky stools, add Dahuang; for shortness of breath and weakness, add Huangqi and Dangshen; for liver heat and dizziness, add Zhizi; for insomnia, add Suanzaoren.

Indications: Obesity-related fatty liver.

Liver-Gallbladder Damp Heat Type

Symptoms: Fatty liver on B-ultrasound, accompanied by chest and rib pain, dry and bitter mouth, irritability, chest tightness, obesity, loss of appetite, and possibly jaundice and yellow urine in severe cases.

Formula: Chaihu Erling Decoction

Ingredients: Chaihu, Huangqin, Banxia, Cangzhu, Houpo, Chenpi, Fuling, Zhuling, Ze'xie, Baizhu, Yujin, Chuanlianzi, Zhizi, Yinchen,Heye.

Indications: Obesity-related fatty liver.

Liver Depression and Spleen Deficiency Type

Symptoms: Aching and pain in both sides, fullness and distension in the abdomen (worse after eating), loose stools or indigestion, nausea, vomiting, irregular menstruation in women, shortness of breath, fatigue, pale or dark red tongue, thin white coating.

Formula: Chaihu Sharen Decoction

Ingredients: Chaihu, Yujin, Danggui, Baishao, Baizhu, Fuling, Sharen, Xiangfu, Dangshen, Huangqi, Gancao, Shanzha, Mugua, Bohe,Yinchen, Heye.

Modifications: For severe side pain, add Chuanlian; for abdominal distension, add Chuanpo and Zhike; for loose stools, add Cangzhu; for nausea and vomiting, add Zhuru.

General Type

Symptoms: Rib pain, obesity, irritability, occasional low mood. Unclassified individuals.

Formula: Erling Erbai Decoction

Ingredients: Zhuling, Fuling, Baizhu, Ze'xie, Shanzha, Danshen, Chenpi, Zhuru, Chaihu, Baishao, Chishao, Chuanlianzi, Cangzhu, Yinchen, Heye.

Indications: Various types of fatty liver.

Modifications: For dry stools, add Dahuang and Juemingzi. For kidney deficiency, add Gouqizi and Maidong. For soreness and weakness of the lower back and knees, add Chuanduan and Niuxi; for dizziness and blurred vision, add Juhua; for insomnia and vivid dreams, add Suanzaoren and Yuanzhi; for restlessness and heat, add Mudanpi and Zhizi.
